Our Company

Access To Community was founded in 2019 to provide quality services to New Jersey adults with intellectual and developmental disabilities ages 21 and older. We are here to guide our clients in living as independently as possible through programs funded by the New Jersey Division for Developmental Disabilities/Medicaid. With several years of experience working as a Support Coordinator, the CEO/SCS has worked with and provided enormous quality support for individuals with developmental disabilities across the state of New Jersey to live fulfilling lives.

Support Coordination is an NJ Division for Developmental Disabilities funded program that assists eligible New Jersey adults with intellectual and developmental disabilities, ages 21 and older, who live in a provider-managed setting like a supervised apartment or a group home, or who live in their family home or their own apartment or home. The main goal of this program is to give these individuals the support that they need to live independently.

Support coordination services allow you or your family member to receive essential support and services, and our team is proud to assist you and/or your family member in planning and connecting to supports and services that meet and address your needs and preferences.

Our Mission

Our mission is to serve individuals with Developmental and Intellectual Disabilities and their families, to increase their total independence by connecting them with quality services and supports to ensure meaningful participation in their communities and exercise their right to make choices.
